FHSAA REQUIRED FORMS -- To participate in any MCC Athletic Sport, the Florida High Schoo Athletic Association requires the following General Eligibility forms to be kept on file by MCC.
Preparticipation Physical Evaluation - This completed form must be kept on file by the school. This form is valid for 365 calendar days from the date of the evaluation as written on page 2.
Concent and Release from Liability Certificate - This completed form must be kept on file by the school.
